Assessment Task

You are to read two case studies and complete the questions for each.

Assessment Task 3: Role plays

You are to develop a behaviour management and review plan for the client from role play 2 in Assessment Task 3.

Assessment Task 6: Project – working with clients with dementia

You are to monitor two clients with dementia for three days and complete a journal for each client.

How to submit your assessments

When you have completed each assessment task you will need to submit it to your assessor.

Make sure you photocopy your written activities before you submit them – your assessor will put the documents you submit into your student file. These will not be returned to you.

It is really important for a person suffering from dementia to have medication .Deliberate effort of withholding medication is an neglect . which should be reported to manager who knows or is involved in the client’s circle of support.

Person centred care is a type of treatment and care provided that involves the person with their care and considers the need of the older person carers. It is beneficial because carer get to know patient very well , care can be provided which is specific to patients need and therefore provide better care . It also promotes and facilitate patient’s responsibility, patients are more likely to engage in treatment .Patient feel more supported to make behavioural changes and also feel empowered to self manage .It makes a positive difference to health outcomes and patient satisfaction .

Repetitive behaviour

Incidence in past and objects at home might be reason of their repetitive behaviour. For example, when the person sees coats, hats and gloves, they may react by repeating statements about going home. It is important to be alert for these triggers. Observe the behaviour and keep notes.

Paranoia

Watch the person for signs of increasing anxiety. The first sign of increasing anxiety and potentially aggressive behaviour is an increase in movement.

See if you can find an immediate cause:
1. Are they afraid of something that just happened?

• Reduce noise and activity levels. Try to keep a balance between restful and active periods.

• Maintain a consistent daily routine. Avoid changes to both routine and environment.
