One of the treatment goals is to help Bonnie confront her fears in order to reduce her symptoms and your supervisor has asked if you could plan activities which will:

1. Help Bonnie feel more comfortable in public.

Keeping a journal or record This is a continuous process and Bonnie needs to be encouraged to keep track of her personal life activities and feelings and mental conditions attached to the same. This needs to be achieved within 1 month Keeping track on the situational feelings and the outcomes of facing a social situation may help Bonnie to understand the reasons leading her to embarrassment and those can be used for personal development. As influenced by Keuroghlian et al. (2015), the personal record may also help the health care professional to plan activities that can reduce stress.
Prioritizing issues As per the views of Isomura et al. (2015), stress can be induced through poor time management and allocating resources inappropriately. Therefore, Bonnie would be assisted for managing her time and energy so that she can complete her tasks within time effectively. This needs to be achieved within 2 months Effective time management can help reduce anxiety. For instance, Bonnie can prepare earlier she needs to present something in the classroom. Managing her time for studies would help her to prepare well that can reduce her anxiety or stress.
Avoiding unhealthy substance use Bonnie would be encouraged not to abuse substances for avoiding stressful situations as substance abuse may cause serious health problems. This needs to be achieved within 3 months Reducing the use of substances may help to keep Bonnie physically healthy. This is required to adapt to stressful life situations.
Communication and interaction skill development As Bonnie find it difficult to communicate effectively in social situations developing communication skills may help her interact with her peers and other persons This needs to be achieved within 4 months Effective communication helps to increase social support that is very important for managing stress. It is due to the fact that Bonnie can approach her friends or other people while she is facing problems related to anxiety and stress.

B. How would you ensure all services are aware of the meeting?

An email is to be sent to all the people included in the meeting along with phone calls.

Another goal is to help Bonnie planning her daily works so that she can cope with the problems due to her mother’s illness.

E. Should Bonnie be at the meeting?

The case work process will be monitored through assessing the changes that can be observed within Bonnie. Bonnie can be used to develop a record on her daily meetings with people. Along with this, Bonnie can record her activities she has applied for managing her household and sibling’s study.

  • Develop a case management profile for two different potential clients who are requiring case management.

Brief description of your agency The agency is responsible for providing care services to the clients that are suffering from psychological illness such as depression, anxiety, phobia and other behavioural symptoms.
Background information The client has been observed not to be attentive in the classroom. The client has reported of having recurrebnt thoughts about committing suicide. Along with this the clients does not enjoy pleasurable activities and does to visit friend home after several requests from her friends.
Risk factors The symptoms that are observed to be present within the client may be an indicator of having clinical depression. One of the major risks associated with suicidal ideation.
Medical history No significant medical history is present
Current physical wellbeing The client faces difficulties in completing household work due to laziness.
Employment status Unemployed
Accommodation The client resides with her family
Familial relationships The client lives with her mother and has no siblings
Culture
Specific communication needs The client finds it discouraging to talk with her friends.

a. What you would need to consider when allocating the case to a worker

The qualification of the case worker and their experiences in dealing with clients with mental illness is to be considered. Along with this, the location of the case worker is needed not to be in huge distance as they may be required to provide essential services in an emergency situation.

  • Services such as dietary plan are needed to be provided for helping the clients to maintain a healthy lifestyle.

  • The clients are to be invited to the consultation session. After establishment of rapport the clients are to be provided with some tasks that would help them to identify the problems that can hamper their wellbeing. And solutions are to be found collaboratively.
