Assessment of Social Determinants of Health
There are various tools and measures recommended by Centers for Disease Control and Prevention (CDC) for putting SDOH into action. They are designed to address public health problems and concerns, such as tobacco use, cancer control, diet and activity, violence prevention, obesity disparities and certain diseases like strokes, diabetes, HIV and asthma. For example, PRAPARE Implementation and Action Toolkit compiles resources, best practices, and lessons learned from health centers focused on how to implement a SDOH data collection initiative. The toolkit is accompanied by an assessment tool. (RHIhub, 2020) The tool was developed based on SDOH that consists of a set of core measures to help complete assessment.
