No, there is no patients detailed information seen, there is only smoking status, if there were charts with the detailed information like Ernesto’s then we could quantify several patients. If history and physical notes were detailed like Ernesto’s then we can find the data used or given to patience to quit smoking.

15. The CDS Smoking Cessation protocol calls for every patient’s current smoking status to be assessed and documented in the EHR. Do you think this protocol was followed over the last 12 months? Please explain your answer.17. What are your recommended changes related to meeting Goal 1 next year?
In order to meet the goal next year, I would have all patients fill out a form upon arrival and include their smoking status. For ease, they may check mark or circle “current, former or never ” options. This can be documented later in the EHR when evaluating them and inputting the answers given by patients into the system. If this method doesn’t work, the EHR may have a smoking status section in which a “Check box” method can be used for“Current, former ornever”. They can make this section as a required field (hard stop) where they cannot be bypassed and can’t move to the next page until this section is answered and completed.
Goal 2 cannot be met until goal 1 is accomplished. Once a patient is determined if they are a smoker, non-smoker, or former then an additional field can be organized if they check mark a box for smoker.
