- By giving relevant information on the areas of health, care, and welfare issues relevant to the older person and their family or carers, a person can assist an older person in accessing community support agencies. You can also discuss the various alternatives accessible to them through support agencies and assist the older person, family, or caregivers in contacting the appropriate agencies. You could also assist the elderly person, their family, or their caregivers in determining their specific assistance needs from community support agencies and establishing the necessary services or support.

Mary is an 83 year old lady who has been diagnosed with multi infarct dementia. Mary lives with husband - Jack, and the couple are supported by family members. Jack reports that Mary is getting worse and is frequently waking up at night and getting up. She misplaces items and then accuses family members of taking them.
Over the last few months she has started to see young people in the trees in the garden. She is worried in case they fall out.
